I hereby release Kona Pain Institute LLC and associates from all claims and liabilities arising from the use of such modalities, indemnifying and holding the above harmless from all liabilities and claims therefrom, whatsoever.
ANSSR therapy facilitates comfort and relaxation. Physiologic self-regulation can be extensive. All benefits are spontaneous effects of relaxation, dictated by individual needs. ANSSR therapy is not intended to diagnose, treat, cure, mitigate, or prevent any specific disease.
ANSSR therapy is safe, non-invasive, and relaxation-oriented. Neither Arneson nor Kona Pain Institute LLC is responsible for any adverse conditions or adverse changes in my physical or emotional health.
I take full responsibility for my safety on the office property, outdoors and indoors.
Arneson is not a physician and makes no attempt to undermine the advice of my physicians.
I am free to discontinue these applications at any time.
ANSSR therapy requires total comfort for both client and practitioner. In the unlikely event that a client's conduct is incompatible with the method, Arneson reserves the right to refuse or discontinue treatment at any time.
No shows will not be offered another appointment at KPI. Kindly make online scheduling changes or cancellations at least two days before your appointment.
Client agrees to a single video interview, at the office, without remuneration, when noteworthy changes occur. For convenience, the interview will be recorded immediately after an ANSSR treatment.
Client may decline the interview for any reason.
Only the client's first name is used in the interview.
Veterans will be asked for branch of military service, service era, and MOS only, but may provide any additional information.
The video will be available for unlimited public viewing, for educational purposes and KPI outreach.
If needed, client may receive additional ANSSR treatments following the interview.
Client may receive a request for followup data and/or testimonial several weeks after the interview.
